The forms will be built into the WCCIS platform for electronic use (with a word document also available).
The form will be built to ensure that questions are only displayed when relevant to the decision. For example, the best interest section of the form will appear if a person has been assessed as lacking capacity to make a specific decision.
Please review the proposed BIA and MCAA forms attached and complete the surveys. Your views and feedback are important to us and will help inform the final documents.
